Job description
Locum Consultant in Elderly Care (Frailty) / General Medicine

This an exciting opportunity to join the Care of the Elderly team at Glangwili General Hospital. This new Locum Consultant appointment will focus on providing an Elderly Care and Frailty service at the "front door" of the hospital, with expertise being concentrated in SDEC, the Emergency Department, the Clinical Decision Unit and Medical Assessment Unit. The successful candidate will work in close conjunction with other Consultant Physicians in the hospital and the community.

Main duties
  • To provide a service in Elderly Care (Frailty)/General Medicine including supporting the development of the COTE service.
  • Work in close co-operation with, and support other clinical, medical professional and managerial colleagues in providing high quality healthcare.
  • To possess experience and knowledge in running a sub specialty service.
  • To provide treatment of medical patients presenting to GGH front door including frailty, Emergency Department, Clinical Decision Unit and Same Day Emergency Care.
  • Providing effective leadership to multidisciplinary staff members in the CDU, SDEC and ED
  • Provide clinical supervision of the SDEC and Emergency Department medical liaison patients.
  • Ensure close cooperation with other medical consultant colleagues to develop seamless pathways of care for patients.
  • Work with the team to the development of "hot clinics" as an alternative to admission.
  • Ensure close liaison with GP colleagues to direct patients to the most suitable service.
  • Ensure development, support and training for nurse consultant and nurse practitioner colleagues.
  • Participating in CPD including audit.
  • Acting as an Educational Supervisor to Resident and Middle Grade medical staff.
  • To participate in the acute general medical on call at Glangwili General Hospital.

Person Specification
Qualifications
  • Full GMC Registration and Licence to Practice
  • Valid Certified Advanced Life Support Skills
  • MRCP or equivalent
  • Appropriate Higher Degree e.g. MD, PhD or MSc or equivalent
  • On Specialist Register/SpR due to obtain relevant CCT within 6 months of interview or working towards CESR /equivalent
Clinical Experience
  • Broad based experience in eg. Acute General and Geriatric Medicine
  • Knowledge of UK hospital systems (or equivalent)
  • Knowledge and participation in CPD
  • Expertise in an elderly based subspecialty preferably frailty
  • Competence and experience in undertaking Comprehensive Geriatric Assessment (CGA)
  • Experience of NHS
  • Wider experience, research and training in providing sub specialty service
  • Membership of the British Geriatric Society (BGS)
Clinical Governance
  • Evidence of participation in clinical audit and understanding role of audit in improving medical practice
  • Knowledge of risk management
  • Knowledge of annual job planning/appraisal review process
Research
  • Experience and knowledge of critical appraisal of evidence so as to improve clinical outcomes
  • Evidence of initiating, progressing and concluding research projects with publication
Teaching
  • Evidence of organising programmes and teaching medical students and resident doctors
  • Willingness to teach all grades of professional multidisciplinary staff
  • Organisation of further teaching programmes in medical education
  • "Training the Trainers" experience
Management
  • Knowledge of the management and structure of the NHS
  • Willingness to participate in clinical management
  • Evidence of management training
