Locum Band 6 Community Mental Health Practitioner (RMN, SW or OT)

We are currently recruiting for a skilled and compassionate Band 6 Community Mental Health Practitioner to join an Adult Community Mental Health Team on a locum basis. This post is open to Registered Mental Health Nurses (RMNs), Qualified Social Workers, or Occupational Therapists with relevant experience in community mental health.

Location: Preston, Lancashire

Contract Type: Locum – Full-Time, Mon-Fri 9-5

Hourly Rate: £26+

Start Date: ASAP

Contract Length: 3 – 6 months (with possible extension)

Role Overview

The successful candidate will manage a caseload of adults with complex and enduring mental health needs, providing assessment, care coordination, and therapeutic interventions as part of a multidisciplinary team.

Key Responsibilities
  1. Manage a defined caseload, providing recovery-focused care coordination under the CPA framework
  2. Undertake comprehensive mental health and risk assessments
  3. Develop and review individualised care plans in collaboration with service users and carers
  4. Deliver evidence-based interventions and monitor treatment effectiveness
  5. Liaise with GPs, acute services, social care, and voluntary organisations
  6. Participate in regular MDT meetings, supervision, and service development
  7. Maintain accurate and timely clinical records in line with trust policies
Requirements
  • NMC registration as a Mental Health Nurse (RMN), Social Work England registration as a Social Worker, or HCPC registration as an Occupational Therapist
  • Minimum of 12 months’ post-qualification experience in community mental health services
  • Knowledge of relevant mental health legislation (e.g., MHA, MCA, safeguarding)
  • Excellent clinical assessment, communication, and organisational skills
  • Full UK driving licence and access to a vehicle (community visits required)
  • Enhanced DBS check (mandatory)
Desirable Skills and Experience
  • Experience using electronic patient records (e.g., RiO or SystmOne)
  • Training in psychosocial interventions or CBT approaches
  • Familiarity with trauma-informed or recovery-oriented care models
