Job summary

Locum Ready is seeking an Advanced Clinical Practitioner to support regional community health and primary care services across Cardigan. This flexible, sessional role suits ACPs who can work autonomously in diverse community clinics, delivering advanced assessments and urgent care.

You will join established clinical teams, perform ACP-led sessions, triage urgent demand by phone or in person, and prescribe safely within your scope.

Qualifications

• Must hold Advanced Clinical Practitioner qualification (Level 7).
• Experience in GP, urgent care or community health services.
• Independent Prescribing (V300) desirable.

Responsibilities

• Deliver ACP-led sessions within community and primary care settings.
• Managing acute, undifferentiated and minor illness presentations.
• Supporting urgent patient demand through telephone or face-to-face triage.
• Prescribing safely within scope of competence.
• Managing clinical actions including results, medication reviews and follow-up tasks.
• Recording accurate and clear digital documentation.
