Job summary

Locum Ready is seeking an Advanced Clinical Practitioner to support regional community health and primary care services across Porthmadog. This flexible, sessional role suits ACPs with autonomous clinical assessment experience and the ability to respond to urgent patient demand in community clinics.

You will work within established clinical teams, delivering ACP-led sessions, managing acute and minor illness, and performing safe prescribing within your scope.

Qualifications

• Advanced Clinical Practitioner qualification (Level 7) required.
• Nursing, Paramedic Science, AHP or similar clinical background essential.
• Independent Prescribing (V300) desirable.

Responsibilities

• Deliver ACP-led sessions within community and primary care settings.
• Manage acute, undifferentiated and minor illness presentations.
• Support urgent patient demand through telephone or face-to-face triage.
• Prescribe safely within scope of competence.
• Manage clinical actions including results, medication reviews and follow-up tasks.
• Record accurate and clear digital documentation.
