Discharge and Flow Team - Empowering Independence


Reablement is a short-term, therapist-led service focused on promoting independence to help individuals break free from ongoing services. Our team provides information, needs analysis, equipment, adaptations, and targeted support.


Since March 2020, we have collaborated with health services to transform hospital discharges in Wiltshire. The Wiltshire Locality Hub manages discharges seven days a week from acute hospitals and facilities beyond the county borders.


This role offers variety and the opportunity to work within a small, flexible team that supports each other to complete tasks.


As a Locality Coordinator, you will be at the core of our Wiltshire Locality Hub, coordinating hospital discharges alongside three main acute hospitals and NHS and health partners. The service operates seven days a week, and weekend work is required.


We are looking for candidates with a background in health or social care, especially those experienced in supporting adults in their homes. Knowledge of adult care legislation, safeguarding guidelines, and social care transformation is essential.
