Job summary

Betsi Cadwaladr University Health Board (BCUHB) North Wales is seeking an experienced Local Primary Mental Health Practitioner (Band 6) to join the Alltwen Community Mental Health Team in Tremadog, Gwynedd. The role involves initial assessments, time-limited interventions, and close multi-disciplinary collaboration.

The post supports a busy primary mental health service with opportunities for professional development, equality and diversity commitments and family-friendly policies.

Qualifications

• NMC registered Nurse (Part 3 or Part 13), social care Wales or HCPC.
• Up-to-date working knowledge of the Mental Health Act (1983) and Mental Health Wales Measure 2010.
• Evidence of continuing professional development.
• Knowledge of consent, data protection, patient confidentiality and risk management with health and safety issues.
• Knowledge of national strategies related to nursing and of Information technology and its application.

Responsibilities

• Provide initial mental health assessments in line with BCUHB procedures for LPMHSS.
• Provide time-limited interventions in primary care settings for patients with mild to moderate mental health problems.
• Act as the first point of contact for GPs, offering advice, support and signposting to appropriate agencies.
• Manage therapeutic care/interventions within the clinical environment and ensure quality of care.
• Welsh language ability is desirable; Welsh and/or English speakers are welcome to apply.

An exciting opportunity has arisen in Primary Mental Health Services for an experienced, motivated and forward thinking practitioner to join a busy supportive team based in the West area.

Applicants with a professional registration in occupational therapy, social work and nursing are welcome to apply.

Position has arisen with the South Community Mental Health teams at an exciting time where services are under review and development in order to meet the changing needs of both patients and carers.

The successful candidate whilst working closely with a multi – disciplinary team, will receive referrals directly from GP’s and provide short term interventions for individuals with mild to moderate illness and will work closely with colleagues from secondary care and specialist services as well as third sector organizations and statutory agencies.

The successful candidates will manage a caseload of patients with whom they will work alongside to formulate risk management plans and care planning based on robust assessment of need.

We are looking for a good team player who has excellent communication skills and the ability to prioritize and manage their time effectively.
