Job summary

NHS Jobs is seeking a Local Primary Mental Health Practitioner to join our primary care teams in Wales. The post holder will assess and provide time-limited interventions for patients with mild to moderate mental health problems and act as the first point of contact for GPs, offering advice and signposting to appropriate agencies.

The role requires managing therapeutic care within the clinical setting and maintaining accountability for the quality of care delivered within the designated area.

• Provide initial mental health assessments in line with the agreed BCUHB assessment procedures for the LPMHSS.
• Provide time-limited interventions in the primary care settings for patients with mild to moderate mental health problems.
• Act as the first point of contact for GPs, to offer advice and support and signposting on to appropriate agencies.
• Manage their own therapeutic care/interventions within the clinical environment and be accountable for the quality of care they deliver within the clinical area that he or she is responsible for.
