Job description
Overview

An exciting opportunity for a senior mental health practitioner to work within the Living Well service in Tameside & Glossop.

The post involves working collaboratively with individuals in a community setting to help them navigate and connect to a range of mental health services, community activities and services in line with their individual needs, circumstances and goals to enhance their health, wellbeing and quality of life. The role includes offering initial conversation and biopsychosocial assessments, and collaboratively engaging in person-centred next step planning. It also involves screening referrals that come through the open door and, if required, devising risk management plans with service users as part of the Duty function. This pathway promotes self-management and provides advice to colleagues where necessary.

As a SMHP you will be able to offer brief interventions (maximum 12 weeks) including devising coping strategies, problem-solving, motivational and behavioral activation work, anxiety management, risk management and suicide prevention work, medication support, promoting social and interagency support, providing psychological support and biopsychosocial education aimed at insight development, promoting self-help through resources and signposting, and reviewing treatment plans with service users deemed too complex for other parts of Living Well Team or who may not yet meet criteria for secondary care services.

  • Develop risk management plans as part of Duty function where required.
  • Deliver brief interventions (up to 12 weeks) including coping strategies, problem-solving, motivational work, behavioral activation, anxiety management, risk management and suicide prevention.
  • Offer medication support where appropriate and promote social and interagency support.
  • Provide psychological support and biopsychosocial education aimed at insight development and self-management.
  • Signpost and support self-help through resources; review treatment plans for complex cases not meeting secondary care criteria.
Qualifications and Experience
  • Applicants must possess RMN / RNMH / Social Worker / OT or equivalent.
  • Completion of Preparation for Mentorship / Practice Teacher is desirable, with significant post-registration experience.
