A healthcare provider in the UK is seeking a dedicated Living Well After Stroke Team Support Worker to support stroke patients in their recovery. This role involves maintaining a large caseload, providing holistic reviews, and collaborating with healthcare professionals. Candidates must have at least NVQ3 or equivalent, along with strong communication and health coaching skills. The position offers flexible working options and a generous benefits package.
This role is to provide personalised practical support, under the guidance of qualified members of the team to people who have had a stroke, and their families and carers. The postholder will be expected to work autonomously in a range of clinical settings across all acute and community settings. The post holder will help patients to review their rehabilitation progress and plan how to maintain and build upon their recovery incorporating principles of supported self management and health coaching techniques. They will have comprehensive knowledge of tertiary charity and community based providers who may support the person and their family and carers in living well after stroke. The post holder will be responsible for maintaining a large caseload and ensuring that six month post stroke reviews are provided in a timely manner in accordance with the Stroke Sentinel National Audit Programme.
The main duty in this role is to maintain a large caseload of people approaching six months post stroke and provide a holistic review of their recovery. You will also liaise with professionals across the stroke recovery pathway to promote effective communication and coordination of stroke patients across primary secondary and tertiary care services as required.
