Position Overview

This is a full-time remote position, primarily based in Central MA, CT, or Western NY, with occasional onsite meetings or training. The role involves remote patient monitoring and care coordination for patients with chronic conditions.

Salary Range

$60,000 - $62,000 USD per year, based on experience and location.

Key Responsibilities
