Minimum Qualifications:

Degree: Certificate. Qualification of Licensed Practical Nurse (LPN). Proof of graduation from an approved National League of Nursing LPN program.

Education: Graduate from an accredited community college, junior college, college, university, or vocational nursing program approved by the appropriate State agency and accredited by one of the following accrediting bodies at the time the program was completed: The National League for Nursing Accrediting Commission (NLNAC), the Commission on Collegiate Nursing Education (CCNE), the Commission on Nursing Education Accreditation (CNEA), or another acceptable accreditation body outlined in the TO.

Experience: Experience in Health Care Administration or in case management, social work, or family advocacy for at least 24 months within the past 48 months.

Licensure: Current, full, active, and unrestricted license as a practical or vocational nurse.

Required Skills/Abilities:

• Initiate, perform, and complete assigned duties in providing care to diverse patient populations.
• Complete patient care based on patients’ conditions, use judgment in selecting procedures and treatments, and accurately report and record relevant patient information.
• Observe, identify, and respond to patient needs, including medication administration, equipment-assisted care, and patient/family education.
• Prepare and administer prescribed medications (oral, topical, subcutaneous, intramuscular, and/or intravenous) and perform treatments as per policies.
• Recognize urgent or emergent situations, seek assistance, and initiate emergency interventions as directed.
• Support complex diagnostic tests and procedures, including preparation, assistance, and monitoring.
• Orient peers and staff to policies, skills, and standards of practice.
• Promote a healthy work environment to improve healthcare delivery, patient and family education, and staff satisfaction.
• Perform LPN services supporting the Referral Management Center (RMC) as required.
