Job Title : Leaving Care Worker

Location : Lincolnshire County Council

Contract : Full-time, Initial 3-month contract (with rolling extension)

Driving Requirement : Full UK Driving Licence and access to a vehicle essential

Role Overview :

Lincolnshire County Council is seeking a dedicated Leaving Care Worker to act as a key point of contact and coordination for young people transitioning from care into independent adulthood. This vital role supports care leavers aged 16 to 25, ensuring they receive the emotional, practical, and professional support necessary to thrive.

You will lead on Pathway Planning, working closely with young people, their families, peers, and a network of agencies to build tailored support packages. This role is about more than case management—it is about empowering, challenging, and guiding young people to achieve their potential and foster lasting positive change.

Duties of the Role :

  • Building trusted and respectful relationships with young people
  • Developing and reviewing Pathway Plans
  • Carrying out home and community visits
  • Coordinating agency support and delivering targeted interventions
  • Supporting young people to access housing, health, education, and training

The Ideal Candidate Will Have :

  • Experience working with young people and their families
  • Understanding of the leaving care agenda (16–25 age range)
  • Full UK driving licence
  • Enhanced DBS
  • Ability to work independently and as part of a team
