Job summary

Job Title: Leaving Care Specialist
Contract Type:
Full-Time (37 hours/week) – Temporary
Pay Rate: £15.93 per hour (PAYE)
Location:
BradfordJob role: The Leaving Care Specialist (Personal Advisor) supports young people aged 16–25 as they transition out of care, offering practical, emotional, and advocacy support across key areas such as housing, education, employment, and wellbeing. The role focuses on empowering young people to build independence while working closely with families and professionals to provide coordinated, person-centred support.

Key Responsibilities

  • Support care-experienced young people (16–25) in areas like housing, education, employment, health, and wellbeing.
  • Complete needs assessments and develop/review Pathway and Action Plans.
  • Provide practical, emotional, and advocacy support.
  • Facilitate group work, drop-ins, and life skills development.
  • Collaborate with families and multi-agency professionals.
  • Maintain accurate records and attend reviews and court as needed.
  • Support smooth transitions into adulthood and independence.
Essential Skills & Experience
  • 2+ years’ experience working with young people.
  • NVQ Level 4 or equivalent qualification.
  • Strong communication, engagement, and advocacy skills.
  • Knowledge of safeguarding, adolescent development, and multi-agency working.
  • Proficient in using electronic case recording systems (e.g., LCS).
  • Fluent spoken English (advanced level).
  • Flexible, team-oriented, and able to work independently.
  • Full UK driving licence and access to transport.
