Job Title: Personal Advisor – Leaving Care Team
Location: South Tyneside
Contract: Full-time | Temporary (with potential for extension)

Are you passionate about making a positive impact on the lives of young people leaving care? We're looking for a dedicated and empathetic Personal Advisor to join the Leaving Care Team in South Tyneside.

In this rewarding role, you’ll support young people aged 16–25 as they transition from care to independence. You’ll be matched with care leavers over the age of 18 and work as their lead professional—carrying out assessments, helping them create and follow pathway plans, and offering both practical and emotional support. You’ll also work with young people aged 16–17 to build trust and begin preparation for life after care.

This is a front-line, relationship-based role requiring flexibility, strong communication skills, and a commitment to putting young people at the heart of your work. You’ll work closely with colleagues in social care, education, housing, health and voluntary sectors to provide coordinated, person-centred support.

Key responsibilities include:

  • Providing structured support to young people leaving care, including help with accommodation, employment, education, life skills, and emotional wellbeing.

  • Undertaking assessments and developing personalised pathway plans.

  • Responding to crises and working creatively to resolve complex issues.

  • Recording your work accurately and contributing to the team's performance and statutory obligations.

Who we’re looking for:

  • Experience working with vulnerable young people, ideally within a leaving care, youth work, or social care setting.

  • An understanding of the issues facing care leavers, and a commitment to improving outcomes.

  • The ability to work independently, while also contributing to a wider team approach.

  • A flexible, resilient, and proactive mindset, with strong interpersonal skills.

Additional information:

  • An Enhanced DBS will be required.

  • The role may include working outside standard office hours to meet the needs of young people.
