Job summary

Pennine Care NHS Foundation Trust is seeking a Learning Disability Occupational Therapist (NHS AfC Band 5) for a permanent full-time role (37.5 hours/week) based at Hollingworth Clinic, Hyde. The post involves autonomous community-based OT work with adults with learning disabilities, including complex needs such as autism.

You will join an established learning disability team, with opportunities to develop clinically and professionally in a multi-disciplinary environment.

Responsibilities

• Provide occupational therapy within a community team for adults with learning disabilities.

An exciting opportunity has arisen for a band 5 occupational therapist to join the Tameside Adult Community Learning Disability Care Hub on a permanent full time basis.

This post would suit a newly qualified Occupational Therapist.

The successful candidate will be a highly motivated, person centred individual with excellent communication skills who will be joining our well established learning disability team.

We aim to provide high quality, community based services for adults with a learning disabilities. As such, we are seeking an individual with a passion for working and improving the learning disability services we provide.

Main duties of the job

We are seeking an enthusiastic and well-motivated occupational therapist to work as part of our multi-disciplinary community team providing services to adults with learning disabilities. The post holder will hold a caseload and work autonomously. They will use a variety of approaches and interventions with service users who have complex needs including autism. Previous knowledge and experience of working with people with learning disabilities would be an advantage. The community team consists of a senior OT, physiotherapists, speech and language therapists, nurses, psychologist, psychiatry, children and young people's team and autism diagnostic service. The post holder will visit clients in their own homes, day services, residential care homes and other community settings.

Detailed job description and main responsibilities

To undertake occupational therapy assessments addressing occupational performance, strengths and skills deficits, enabling the service user in areasof self-care, productivity and leisure. To collaboratively identify the goals of occupational therapy intervention withthe service user as part of the overall care plan. To plan and implement individual and/or group evidence-based interventions,in collaboration with the service user, using graded activity to achieve therapeutic goals. These interventions may take place in a variety of settings including hospital,community and patients home. To monitor and evaluate treatment in order to measure progress and ensure effectiveness of the intervention. To provide support and education to service users and carers regarding aspects of occupational therapy. To attend appropriate case conferences, clinics, referral and review meetings,contributing to decisions with regard to service user care plans. To assess and actively manage individual activity and environmental risk to ensure the safety of the service user, self, colleagues and wider public, whilstrecognising the right of the service user to take positive risks in promoting their recovery. To ensure that up to date written and electronic records are maintained in accordance with Professional and Trust standards. To take responsibility for prioritising own clinical case load.

Education and Qualifications
