Job description

Learning Disability Crisis Response Intensive Support Practitioner

We have an exciting opportunity within the team for two registered professionals to join the Learning Disability Crisis Response and Intensive Support Team as a Band 6 practitioner—one post at 37.5 hours a week and another at 22.5 hours, or two posts at 30 hours each.

The team provides a service from Monday to Sunday, 8am-9pm, including bank holidays, based at the Agnes Unit. The successful candidates will have the opportunity to discuss flexible working patterns within these hours.

You will manage your own caseload and work within a specialist team comprising nurses, healthcare support workers, Occupational Therapists, Speech and Language Therapists, Psychiatry, and Psychology, with input from allied health professionals, as part of the wider Learning Disability Service.

This team provides support, clinical assessments, interventions, and risk management for individuals with learning disabilities at risk of inpatient admission or community placement breakdown.

Support is delivered through Positive Behavioural Support and intensive support for individuals with mental health needs and a learning disability diagnosis.

The service covers Leicester, Leicestershire, and Rutland. A full driving license and access to a vehicle insured for work purposes are required.

Main duties of the job

  • Manage a caseload of patients with complex needs in the community, conducting specialist risk assessments and interventions.
  • Triaging new referrals from various sources, including patients, carers, and professionals.
  • Provide clinical assessments, support, planning, and oversight for patients at risk of admission or placement breakdown.
  • Act as the lead professional, completing assessments and maintaining accurate records.
  • Collaborate with inpatient teams, commissioners, and other case managers.
  • Support Community Learning Disability Teams with high-risk cases through wrap-around support or advice.
  • Offer training and advice to carers, providers, and health and social care staff regarding individual needs and Positive Behavioural Support Plans.
  • Provide specialist advice and support to help individuals remain in the community and prevent inpatient admissions where possible.
  • Deliver direct interventions when required.
  • Monitor and support cases using pathways such as the Mental Health and Positive Behavioural Support Pathways, until risks are manageable, then hand over to LD community teams for ongoing monitoring.
