Job description

Job Overview

We have an exciting opportunity within the team for two registered professionals to join the Learning Disability Crisis Response and Intensive Support Team as a Band 6 practitioner—one post at 37.5 hours a week and one at 22.5 hours a week, or two posts at 30 hours a week. The team provides a service Monday to Sunday, 8am to 9pm, including bank holidays, based at the Agnes Unit. We are open to discussing flexible working patterns within these hours. The successful candidates will manage their own caseload and work within a specialist team comprising nurses, healthcare support workers, Occupational Therapists, Speech and Language Therapists, Psychiatry, Psychology, and allied health professionals, forming part of the wider Learning Disability Service.

The Learning Disability Crisis Response and Intensive Support Team offers support, clinical assessments, interventions, and risk management for individuals with learning disabilities at risk of inpatient admission or losing community placements. Support is delivered through Positive Behavioural Support and intensive mental health interventions for individuals with complex needs living in Leicester, Leicestershire, and Rutland.

This position is ideal for a registered professional with relevant experience working with people with learning disabilities. Experience with high-risk, complex cases and community work is desirable. This role offers an opportunity to develop clinical skills and work within a passionate team dedicated to enabling individuals with learning disabilities and complex needs to live successfully in the community.

The successful candidates will receive a comprehensive induction and access to bespoke training related to assessment and risk management for patients with learning disabilities and complex behavioral needs.

You must hold a full driving license and have access to a vehicle insured for work purposes.

  • Manage a caseload of patients with complex needs in the community, including risk assessments and interventions.
  • Triaging new referrals from various sources, including patients, carers, and professionals.
  • Provide clinical assessments, planning, and oversight for at-risk patients in the community.
  • Act as lead professional, completing assessments and maintaining records.
  • Collaborate with inpatient teams, commissioners, and other case managers.
  • Support Community Learning Disability Teams with high-risk patients through wrap-around support and advice.
  • Offer training and advice to carers, providers, and health and social care staff, especially regarding Positive Behavioural Support Plans.
  • Provide specialist advice and support to help individuals remain in the community and prevent inpatient admissions where possible.
  • Deliver direct interventions when required.
  • Monitor and support patients using pathways such as the Mental Health Pathway and Positive Behavioural Support Pathway until risks are manageable, then hand over to LD Community teams for ongoing monitoring.
