Description

Location: Hexham, Northumberland

Pay: £13.21 per hour (£25,838 per annum [pro rata]) and great benefits including Health Cash Plan

Typical day as a Lead Support Worker (known as Coordinator)

  1. Creating support plans with customers and coordinating a small team of Support Workers.
  2. Helping customers with daily activities such as making meals, shopping, budgeting, maintaining tenancy, and moving to independent accommodation if able.
  3. Carrying out risk assessments, support planning, goal setting, and reviews.
  4. Engaging in conversations, building rapport, and collaborating with colleagues.

You bring

  • Passion for supporting customers to live their best lives with attention to detail.
  • Experience in creating person-centred support plans and supporting colleagues in their implementation.
  • Experience in coordinating and assessing customer referrals.
  • Ability to work independently, stay calm under pressure, and be resilient.
  • A vehicle insured for business purposes (mileage paid).

Job details
