Job summary

Practice Plus Group is recruiting a Senior Primary Care Nurse on a 9-month maternity cover basis in a category B men's prison setting. The role supports a nurse-led service with an emphasis on patient safety and reduced hospital admissions, including medication rounds and emergency responses.

You will lead a multidisciplinary team, deliver evidence-based care, and continually improve quality with partners and stakeholders, while benefiting from bespoke induction and regional career development.

Qualifications

• Registered nurse with adult scope and current NMC registration.
• Leadership experience essential for multi-disciplinary teams.
• Commitment to ongoing professional development and CPD.
• Understanding of multi-agency working and complex care needs.
• Self-motivated and collaborative team player with a compassionate approach.

Responsibilities

• Lead a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions.
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening.
• Provide outstanding evidence-based care, listen to feedback, and tailor care to what matters to patients.
• Deliver non-judgemental care that makes a real difference.
• Forge links with partners and stakeholders to provide trusted, safe, and proud-to-deliver services.
• Promote best practice, supervision, and continuous quality improvement with shared learning.
• Share knowledge to ensure safe care and support multidisciplinary teams.
