We are looking for a passionate and forward-thinking Primary Care Lead Practice Nurse to lead our dedicated nursing team. This is more than just a leadership role, it's an opportunity to drive innovation, mentor a strong team, and enhance patient care in a supportive and progressive practice.
The Lead Nurse will provide leadership and coordination for the nursing team, ensuring high-quality safe patient care, professional development, and efficient service delivery. The role includes strategic planning and clinical oversight to enhance patient services and optimise workflow within the practice.
Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions, and management and preventative nursing interventions to all patients.
They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and will be part of the Senior Management Team.
This is a permanent role, for 22.5-37.5 hours per week.

Main duties of the jobThe Lead Practice Nurse will provide leadership and support to our nursing team (consisting of 4 Practice Nurses, 5 Healthcare Assistants, and a GP Assistant), and work collaboratively with our Pharmacy Team Lead, GPs, Practice Management, and the PCN Lead Nurse, to ensure the delivery of safe, efficient and evidence-based Long Term Condition management, as well as the full range of primary care nursing duties.
The Lead Nurse will hold CQC lead responsibilities, ensuring services provided by the nursing team are effective, safe, responsive, caring and well-led. They take responsibility for ensuring all relevant policies, procedures, risk assessment and best practice is following within the nursing team, and the wider team for relevant areas.
The role will involve protected leadership time, including attendance at the practice clinical meetings and management meetings, as well as the delivery of planned practice nurse clinics.
About usWe are a friendly and well organised practice, with an increasing patient list of 15,500 patients, operating over three sites; North Shields, Hadrian Park and Tynemouth.
We are a large teaching practice, hosting GP Registrars, F2 Doctors, Pharmacy Pre-reg's, and medical and nursing students.
Our clinical team comprises; 7 GP Partners, 6 Salaried GPs, a Physician Associate, a Paramedic, 4 Practice Nurses, 5 HCA's, a GP Assistant, 4 Pharmacists, and 5 Pharmacy Technicians.
The Practice also hosts a First Contact Physiotherapist, a Mental Health Practitioner, and a Social Prescribing Link Worker.
Our CQC rating is good, and we have high levels of QOF achievement.

Induction Training
