LDA Care Ltd is seeking a Positive Behaviour Support Practitioner to support adults with learning disabilities and autism in their own homes through domiciliary visits. The role requires travel and flexibility to meet individuals' needs, with a salary of £40,000 per year.

You will develop personalised Positive Behaviour Support plans, work with families and carers, and provide training to staff while monitoring outcomes and reporting progress to stakeholders.

Qualifications

• Qualification at Level 3 Health & Social Care or equivalent.
• Experience delivering Positive Behaviour Support.

Responsabilités

• Conduct comprehensive assessments to understand the unique needs and preferences of individuals.
• Develop and implement personalised Positive Behaviour Support plans tailored to enhance each individual's quality of life.
• Collaborate with a multidisciplinary team, including caregivers, families, and other professionals, to ensure a holistic and integrated approach to support.
• Provide ongoing training and guidance to support staff, caregivers, and family members in the implementation of Positive Behaviour Support strategies.
• Utilise evidence-based practices to address challenging behaviours and promote skill development.
• Foster a culture of dignity and respect, valuing the individuality of each person we serve.
• Monitor and evaluate the effectiveness of interventions, adjusting strategies as needed.
• Create easy read Positive behaviour support plans in a way the individual can understand how their needs will be met.
• Systematically collect data on target behaviours and the effectiveness of interventions.
• Analyse data to make informed decisions about the progress of the individual and the need for adjustments to the Positive Behaviour Support plan.
• Regularly monitor and evaluate the effectiveness of the Positive Behaviour Support plan.
• Provide regular reports to stakeholders, ensuring transparency and collaboration in the support process.
