We have exciting opportunities for Band 6 Nurses to work across our PCN Sites within Surrey Downs Health & Care. If you are passionate about caring for people and making a difference within your community, we would love to hear from you!

The post holder is responsible for the ongoing management of patients and/or staff within their clinical setting, working alongside their team lead and taking the chain of command in their absence. This includes assessing care needs, developing, implementing, and evaluating care programs, maintaining standards, and supervising staff as appropriate. The post holder will also facilitate educational programs in collaboration with the Learning and Development Team and work with senior team members to promote patient-focused, research-based practice.

Main duties of the job

We are seeking nursing staff from diverse backgrounds, including experienced nurses seeking a change, those wanting to work locally, or those excited by community-based care with its unique benefits and challenges.

Please note these roles require a current and valid UK driving license and access to a vehicle for work purposes.

PCN - Primary Care Network

Formed from GP practice populations working in partnership to provide local health and care services.

What do PCNs do?

  • Provide ongoing care and support
  • Deploy appropriate staff efficiently
  • Reduce hospital admissions
  • Support people at home, in care homes, and through hospital in-reach
  • Collaborate with community hospitals and specialist teams

Each PCN has a triad of Lead GP/Community, Lead Clinician, and Operational Manager, working with specialist services to ensure a system-wide approach across Surrey Downs.
