Job Overview
The post holder will hold a qualification as an Advanced Nurse Practitioner working clinically within West End Medical Centre, performing clinics and home visits for the patient population, alongside the multi-disciplinary team.
Parallel to this, the post holder will be a Clinical Nurse Leader with previous primary care/General Practice experience, working with the Management Team to identify opportunities for service change and improvement to encourage and enable advancement and development of evidence-based services. They will act as a professional nurse lead providing clinical advice, leadership development, monitoring, and management of all nursing resources within the designated clinical area, facilitating excellent nursing practice and ensuring the delivery of a high-quality, patient-centred service.
They will be instrumental in developing and implementing the Clinical Governance frameworks and in providing professional advice related to service development within the Practice. They will be responsible for planning care coordination and services within the practice and with all stakeholders.

The Nurse Lead is jointly responsible, along with other nurse leads and practice management, for leading and developing the nursing team and non-medical workforce, monitoring and improving quality standards and patient experience, ensuring professional challenge occurs as necessary.
