Overview

The post holder will hold a qualification as an Advanced Nurse Practitioner working clinically within Healthy Prestayn Iach performing clinics and home visits for the patient population, alongside the multi-disciplinary team. Parallel to this, the post holder will be a Clinical Nurse Leader with previous primary care/General Practice experience, working with the Management Team to identify opportunities for service change and improvement to encourage and enable advancement and development of associated evidence-based services. Whilst acting as a professional nurse lead providing clinical advice, leadership development, monitoring and management of all nursing resources within the designated clinical area, facilitating excellent nursing practice ensuring the delivery of a high quality, patient centred service. They will be instrumental in developing and implementing the Clinical Governance frameworks and in giving professional advice in relation to the development of services, within the Practice. They will be responsible for the planning of care coordination and services within the practice and with all stakeholders. To develop the nursing workforce/services that contribute to the delivery of the local service plans, Health and Well Being Strategies, and other national strategies that:
