The post holder for the lead Nurse of the Resilience, Acute Flow and Transformation directorate sitting within the Corporate Division, working effectively with fellow ward teams and Divisional Matrons to deliver quality services for patients, efficiently. There is a specific focus on the deteriorating patient and improving outcomes for patients.

The post holder will lead the Acute Response Team, providing an exemplary service to patients.

The post holder will be responsible for the assessment, planning, implementation and evaluation of care to acutely deteriorating and critically ill patients. This includes non-medical prescribing, ultrasound guided cannulation, arterial blood gas sampling, spirometry out of hours, review of all tracheostomies/laryngectomy patients, liaising with Critical care.

To provide clinical and managerial leadership for nursing and other healthcare professionals.

  • Line manage Acute Response Practitioners and or other support roles within the Acute response team
  • The post holder will provide advice and support to wider members of the multi-disciplinary team.
  • To perform delegated tasks on behalf of the matron in their absence
  • To support with education for Martha’s rule in the Acute Response Team and with wider organisation
  • To ensure risk assessments are completed alongside the matron
  • To facilitate the stepdown of patients from Critical Care to ward areas.
  • To provide informal and formal education to all healthcare professionals within the Trust.
  • To proactively manage the clinical workload at night as necessary.
  • The post holder will be required to rotate between both sites that make up Calderdale & Huddersfield NHS Foundation Trust.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.
