Job description

The post holder will be responsible for implementing the National Policy and Framework for NHS Continuing Healthcare, NHS Funded Nursing Care and Personal Health Budgets for Continuing Healthcare clients on behalf of the DGFT, taking into account the DOH Responsible Commissioner Guidance.

To take the lead role in service development and redesign based on analysis of CHC Service provision together with the ability to drive the service forward. To be responsible for the planning, monitoring and reviewing the continuing care service, providing expert clinical leadership and advise to the continuing healthcare team and wider health and social care economy including community and acute providers and Local Authority

Expected to work autonomously and use own initiative in developing and innovating continuing healthcare services.

Ensuring systems are in place to enable to timely decision making around CHC eligibility and fast track decision and to organise and maintain the momentum of weekly continuing healthcare panels for adults, with appropriate professional representation on panels.

Responsible to ensuring the CHC function is fully supported with up to date standard operating procedures and system policies in line with national directives and organisational instruction. Work hand in hand with the DGFT's safeguarding team to address the safety of patients in commissioned placements.

Main duties of the job

To develop and maintain systems and processes that build on the work of the CHC Team.

To maintain an awareness of legislation and changes to policy guidance affecting NHS Continuing healthcare and Funded Nursing Care.

To support the smooth running of the continuing healthcare and PHB Panels, providing a consistent approach to decision in accordance with the National Framework.

To manage the continuing health care team and PHB panel process to ensure it is efficient, effective, transparent and equitable. Ensuring clients and their representatives have the opportunity to present their case.

To attend monthly meetings with NHSE for CHC/ PHB leads cascading information to the rest of the team and commissioning managers.

To work in accordance with the NMC code of professional conduct maintaining standards of care of current local and national best practice legislation.

To keep up to date with current knowledge and advances in relevant health and social care practice and maintain a clinical creditability ensure that practice is evidence based.

Be responsible for own continuing professional development and interpret appropriate professional clinical guidelines and standards in line with NMS registration and patient safety requirements.

Job responsibilities

Direct contact with patients if undertaking a investigation, by telephone or face to face. Involved with patient in relation to patient safety walkabout in clinical areas and patient safety clinical audit providers.

Will act as a source of expert advice, drawing upon extensive clinical experience in a variety of setting and knowledge of patient clinical outcomes, patient safety and quality improvement initiatives.

To advise provider organisations on appropriate standards and requirements for determining a Primary Health Need and to assist in the development of competent assessors within the organisation

To communicate with national and local organisations, public bodies, patients and carers and independent providers on complex sensitive matters and issues in complex situations arising from the application of NHS Continuing Healthcare criteria and the outcome.

To work in an advisory capacity across the DGFT Commissioning teams to facilitate the implementation of PHBs and the development of functional operating procedures to support this implementation.

Manage unpredictable and challenging situations; acting in a sensitive manner when communicating to patients and/ or their representatives, decisions in relation to eligibility both verbally and in writing, recognising that imparting this information may include settings that are high emotive, emotional and challenging e.g. situations such as at end of life.
