Lead Nurse - Wandsworth End of Life Care Coordination Service (WEoLCCS)
Hours: hours per week. Including weekends and public holidays
An exciting opportunity has arisen to join our WEOLCCS team as a Lead Nurse. Would you enjoy a role working within a team that makes a real difference?
This role plays a vital part in the goal of the end-of-life care system in Wandsworth, supporting patients to achieve their preferred place of care and death, to avoid unplanned and inappropriate hospital admissions and to support rapid discharges from hospital.
The nature of the role will require you to use your initiative and will involve interactions and managing the wide range of complexities presented by patients, their carers and families.

Responsibilities of a Royal Trinity Hospice Lead Nurse
- To be responsible for all aspects of the day-to-day operational Leadership and management for a team of Royal Trinity Carers and Care Coordinators.
- To act as an End-of-life Care champion, build and maintain professional relations (internally and externally) to promote service.
- To work closely with all service delivery teams, Continuing Health Care and care agencies to ensure care is planned to meet the unique and changing needs of patients, their families, and friends.
- To provide an expert clinical resource to the team, the organisation, and external stakeholders.
- To support the Head of Service in developing and implementing strategic goals and direction.
Skills and experience required
- First Level General Registered Nurse.
- A good understanding of the needs of patients with life limiting illnesses and those close to them.
- Experience within the health or social care sector.
- Understands the role of Trinity’s community palliative care service and the wider work of the hospice.
- Understands the challenges of working with people with life-limiting illnesses and those important to them.
- Understands the importance of and confidence to participate in multidisciplinary team discussions.
- Understands the importance of recognising and responding to changes in patients’ condition.
- Leadership and management experience.
- Mentorship and teaching qualification and experience.
- Excellent communication skills and be able to work collaboratively with all members of the multidisciplinary team.
