Role Description

Flourishing Families Assessment Centre are recruiting for support workers to take onthe role as Lead Family Practitioner In the Reading area. Flourishing Families is a part of a leading innovative company, providing Family Court directed Parenting Assessments. This role would require the worker to complete a combination of the follow tasks:

Work directly with the Registered Manager to ensure that Best Practice and a Child Focused approach takes place for the children and families in the Assessment Centre.

Adhere to safeguarding of the children and families within the Assessment Cente.

Ensure that the day to day running of the Assessment Centre is satisfactory and that the safety and wellbeing of the children and families residing in it are priority.

Build a trusting relationship with the child/ren and the family.

To support children and families in a Person Centred and Holistic way, so that Better outcomes can be achieved.

Be able to support a variety of children and families that have a range of complex and challenging needs.

Be the point of contact for the family within the Assessment Centre.

Co-ordinate the delivery of the actions agreed by the practitioners involved, and the family.

to ensure that children and families receive an effective service that is regularly reviewed.

These actions will be based on the outcome of an assessment and recorded in a plan.

Ensure the child/ren and their parents understand what is happening at every point and are encouraged to contribute towards the Family Court directed Assessments.

Ensure the child/ren and familys needs are identified by assessing Assessment Social Worker completing the Parenting Assessment.

Ensure the familys plan is implemented and reviewed frequently (every 6-8 weeks) via the Weekly Case Review meetings.

Coordinate practitioners working with the family to reduce any overlap and inconsistency in the services received.

Additional Responsibilities

Undertake regular reflective and case supervision for staff.

Undertake regular training so that the development of the role Lead Family Practitioner can take place.

Deliver and create existing and new In House Training.

Chair Weekly Case Review Meetings, that cover the cases of the families within the Assessment Centre.

Quality Assure daily reports and review documents.

Undertake On-Call duty on a rota basis that takes place out of hours, including weekends and Bank Holidays.

Prepare reports, attend Multi-Agency and Multi-Disciplinary meetings in relation to the overall wellbeing of the families within the Assessment Centre.

Person Specification

  • Satisfactory clearance of an Enhanced Disclosure & Barring Service (DBS, formerly CRB)
  • Proof of right to work in the UK
  • Ability todemonstrateawareness/ understanding of equal opportunities and other peoples behaviour, physical, social and welfare needs.
  • Good report writing skills.
  • Good English language skills.
  • Ability to travel effectively tovarious locations, driving licence and access to a vehicle is preferred but not essential.
  • Flexibility- You must be willing to work evenings, nights and weekends.
  • Previousexperience would beadvantageousbut not essential as full training will be given.

Note this post is exempt from the Rehabilitation of Offenders Act 1974 (c.53) as it involves contact with children and vulnerable adults.
