Job summary

Tees Esk and Wear Valleys NHS Foundation Trust in Darlington is seeking a Senior Community Mental Health Practitioner to join a multi‑disciplinary team and lead care for people with complex needs in the community.

You will conduct clinical assessments, triage, depot clinics and psychosocial interventions, and provide coaching and caseload supervision to junior staff, while working with partners to promote recovery, safety planning and social inclusion.

Qualifications

• Post-qualification mental health experience.
• Evidence of CPD and professional registration.
• 18 months post qualifying experience including sign-off of appropriate preceptorship or competency framework.

Responsibilities

• Deliver interventions in the community with people who have complex needs.
• Provide coaching, supervision to junior colleagues and wider MDT members.
• Participate in triage, complex assessments, depot clinics and psychosocial interventions.
• Collaborate with partners including statutory and voluntary agencies.
