The Transfer of Care Hub within Colchester General Hospital is looking for a Lead Case Manager to join their developing team.
This is a key role supporting senior case managers and case managers in implementing and promoting the discharge to assess pathway across all divisions. The role involves working closely with discharge and ward teams to standardise processes and improve patient flow, aiming to enhance the patient journey and reduce length of stay.
This is a fixed-term/secondment position until 31st December 2025. Internal applicants employed by ESNEFT will be offered this position on a secondment basis only.
You will delegate and supervise the work of senior case managers and case managers to ensure high standards of service delivery, maintaining responsiveness and quality. You will also support surveys, audits, analysis, or research to aid service development and ensure data quality.
The Lead Case Manager will collaborate with clinical and non-clinical teams across the Trust to develop policies and procedures for seamless patient management across departments. You will participate in recruitment and selection processes for senior case managers and case managers, including shortlisting and interviews.
