Job description

The PCN has a vacancy for an enthusiastic, highly motivated Lead Care Coordinator, covering our Primary Care Network.

The role will involve working closely with the PCN Operations Manager, GP Lead, ANPs, HCAs, MSK staff, and other clinicians who will be providing clinics during the week and on a Saturday.

Main duties of the job

The job role will require excellent IT skills with all MS Office, strong meeting notes dictation, analytical and strong organization skills, as well as the ability to multi-task and communicate with colleagues at all levels in a friendly and professional manner while demonstrating empathy and compassion to our patients.

Place of work will be between the PCN Treatment Centre based at Shelley Manor Medical Centre, Bournemouth BH5 1LX and ECT Office, Southbourne Surgery.

The role will require an Enhanced DBS Clearance. Should you have this certificate as part of the DBS subscription service, we would be happy to accept this providing the original is shown. Otherwise, you will be expected to complete a new application.

Bournemouth East Collaborative Primary Care Network (PCN) comprises four like-minded practices working together in East Bournemouth, Dorset, with a strong reputation for quality improvement and investing in its employees. The PCN serves a population of approximately 55,000 patients with a diverse demographic.

Our Network teams include:

  • Enhanced Care Visiting team for frail housebound patients and those in care homes.
  • BEC Urgent Care Treatment Centre working out of the Private Suite at Shelley Manor Medical Centre.
  • First Contact Physiotherapy Practitioner service.
  • Digital Care Coordinator, Digital Champions.
  • Health & Wellbeing Coaches within Help & Care team.
Job responsibilities
  1. Support PCN Operations Manager with day-to-day operations.
  2. Line Management of PCN Care Coordinators across all PCN Teams including regular 1-1s and yearly appraisals - accompanied by Ops Manager.
  3. Standard return to work conversations for all PCN staff.
  4. Assigning and management of Treatment Centre Clinicians, First Contact Physios, Mental Health Team & Social Prescribers clinical ledgers on S1.
  5. Uploading staff sickness on BrightHR, informing Operations Manager weekly of any issues.
  6. Cover Treatment Centre reception desk when required.
  7. Support additional future service delivery projects as the PCN develops.
  8. Support and assist in the organization of all PCN events.
  9. Teamnet training monitoring for all PCN staff.
  10. Sourcing and issuing of staff uniform - scrubs.
  11. Completion and distribution of Enhanced Care Team visiting rota.
  12. ICE issues - if absence of Digital team.
  13. PCN Website - PCN social media Working alongside Network Administer.
  14. Be responsible for updating the PCN Service booklet.
  15. Develop relationships with social prescribing link workers, health and wellbeing coaches, social services, the enhanced care team, Primary Care Network colleagues, and professionals in the wider health economy as required.
  16. Support the PCN Digital Lead in the deployment and ongoing maintenance of new technologies, remote monitoring tasks-safety nets, and education of patients, i.e. use of Healthcare Apps.
  17. Run regular proactive reports and utilize both S1 and DiiS to identify patients who may be at risk or benefit from new or improved services Population Health Intelligence, public health reports.
  18. Be involved in supporting new processes to bring efficiencies to the Treatment Centre and clinical teams.
  19. Assist with Lantum alongside Operations Manager for booking of Locum GPs.
  20. Provide support to carers to identify and then signpost to support for those in need. Link with the carers leads in the practices.
  21. Development of a PCN Patient Participation group.
  22. Be involved in supporting and promoting cancer screening, e.g. smears, bowel screening for patients not engaged.
  23. To attend and contribute to staff meetings and development meetings constructively.
  24. To identify problems with the operational systems in use within reception and make recommendations for improvement.
  25. Ensure building security has thorough knowledge of entrances, doors, windows, alarm, and security procedures.
  26. Open and close the premises as required following the security procedures.
Health & Safety

Assist with health & safety as required.

Completing documents on BrightSafe as per instruct from Operations Manager.

Qualifications

  • Good Knowledge of SystmOne Primary Care clinical and administrative system. Ability to support and offer training to BEC staff.
  • Proficient IT, analytical and project planning skills.
  • Interest and experience in Digital technologies- public health, health inequalities.
  • Experience and good knowledge of Microsoft Office, Excel.
