Job summary

East:MK Primary Care Network is seeking an advanced nurse practitioner or advanced paramedic practitioner for a full-time role (37.5 hours across 5 days) across four Milton Keynes surgeries. The post holder will work autonomously and with the clinical team, providing expert care and independent prescribing within formulary guidelines.

The role includes varied consultations (face-to-face, tele/ video), home visits, and management of complete care episodes, with opportunities to lead within MDT

Qualifications

• Registered nurse with NMC or HCPC registration.
• Master's degree in advanced practice or MSc pathway eligibility for advanced practice.
• Qualified independent prescriber with current registration.
• Meet NMC revalidation requirements.
• Experience as an Advanced Practitioner with leadership skills.
• Experience across primary care settings including clinical triage.

Responsibilities

• Act as expert practitioner with advanced clinical competence.
• Provide autonomous, patient-centered care.
• Independently prescribe within formulary guidelines.
• Deliver face-to-face, telephone, e‑consult, video consultations and home visits.
• Manage complete episodes of care independently.
• Diagnose, treat, refer and discharge patients using clinical judgement.
• Collaborate within MDT and signpost to meet patient needs.
• Record clear, contemporaneous notes and read coding.
• Contribute to clinical targets (QOF, QIS, ES).
• Explain and initiate appropriate diagnostic tests and investigations.
