We are seeking a compassionate and dedicated Support Worker to join our team, providing high-quality support to individuals with learning disabilities, autism, and mental health needs. This role is central to promoting independence, well-being, and positive outcomes using a person-centred approach, underpinned by Positive Behaviour Support (PBS) principles. Key Responsibilities

• Support individuals in their daily routines, encouraging autonomy and choice.
• Implement personalised care and behaviour support plans, including PBS strategies.
• Provide consistent emotional and mental health support, fostering trust and resilience.
• Promote personal development through structured activities and community engagement.
• Maintain professional communication with individuals, families, and health and social care professionals.
• Accurately document support provided, behaviours, incidents, and progress in a confidential manner.
• Ensure safe transportation to and from activities, appointments, and community outings.
• Foster a safe, inclusive, and supportive environment for all individuals.

Essential Skills and Experience

• Experience supporting individuals with learning disabilities, autism, and mental health challenges.
• Familiarity with or training in Positive Behaviour Support (PBS).
• Excellent interpersonal and communication skills.
• Ability to work effectively both independently and as part of a team.
• Basic IT skills for maintaining accurate records and communication.
• A full driving licence is essential due to the travel requirements of the role.
• A flexible and proactive approach to shift patterns, including evenings and weekends.

Pay Rate

The pay rate is open to discussion and will depend on experience and qualifications.
