  • Working closely with clients, families, local authority services, and placement providers.
  • Collaborating on crisis and contingency plans.

Main duties of the job

Reviewing Section 117 aftercare and care plans for clients residing outside the tri-borough area, working with clients, families, and care providers to ensure effective aftercare planning.

Assessing mental health and learning disabilities, identifying risks and strengths, and developing appropriate aftercare plans.

Providing updates to the ICB lead, raising concerns, and supporting crisis planning.

We are looking for someone with experience in:

The LD Psychiatry service operates across three boroughs, providing psychiatric input into the Tri-Borough LD Community Team. The role offers flexibility to be based in Hammersmith & Fulham, Kensington & Chelsea, or Westminster.

The position involves UK travel and requires effective time management and independent working skills. Clinical supervision is provided by the Trust’s Learning Disabilities Consultant Nurse, and active participation in clinical networks is encouraged.

Review S.117 aftercare plans for clients living outside the area, following an established review process, and develop supportive aftercare plans.

Work with clients, families, and care services to review and advise on Section 117 arrangements, ensuring high standards of care based on evidence-based practice.

  • Registered NMC RNLD and/or RMN / Healthcare professional
  • Post-registration qualification evidence
  • Degree or Diploma level education
Previous Experience
  • Extensive post-registration experience, especially with learning disability services
  • Knowledge of Section 117 reviews and aftercare planning
  • Experience in multi-professional collaboration and partnership working
