An excellent opportunity has arisen to recruit a Social Worker to North Manchester Later Life Community Mental Health Team. The experienced recruit will operate within an integrated, multi-disciplinary team and will mainly undertake commissioning and care purchasing tasks for service users, working within the framework of the Mental Capacity Act 2005. We support practitioners in their professional development and are a friendly and supportive team.

Social work is critical in upholding individuals' human rights, fostering a strengths and asset-based approach to mental health recovery. We are looking for social workers who take an anti-oppressive approach to social care interventions. Social work expertise is a key component of multi-disciplinary integrated working. We have roles enabling you to carry out specific social care duties for individuals. The successful candidate will complete assessments for clients and carers and may also provide care coordination in line with the Trust Care Programme Approach (CPA) guidance and policies. You will be allocated as care coordinator/lead professional for a caseload of both organic and functional clients.
You will participate in the rostered Duty role, which includes initial CMHT assessments of new referrals and feeding back at Multi-Disciplinary Team Meetings.
Complete safeguarding processes as needed, with support from the Team Manager and Seniors.
The Community Mental Health Team operates from Monday to Friday during usual office hours.
