Join to apply for the KWG212068 - Post Diagnostic Support Worker role at NHS Ayrshire & Arran.

An exciting opportunity has arisen for a Dementia Post Diagnostic Support Worker within the Older Adult Service based at Inverurie Hospital within the Aberdeenshire Health & Social Care Partnership. The post is based in Central Aberdeenshire, providing care for individuals referred from GP Practices in the catchment area. Car ownership and a valid license are prerequisites for the post.

We seek a motivated and skilled practitioner with sound assessment skills, good communication within a team, the ability to work independently, and experience in multi-agency working. Experience working with older adults, including those with functional and organic illnesses, is preferred. Patients referred to the service may be under 65 but still qualify for support.

The post closes at midnight on the indicated date unless otherwise stated. Applications may close early due to volume.

The successful candidate will manage a caseload under supervision. Informal visits to the team are encouraged.
