Job summary

The Royal Wolverhampton NHS Trust is recruiting a Band 6 Junior Sister to join the Care Coordination Hospital Inreach team. You will work across our hospital and community sites to lead discharge planning and ensure safe, timely transfers from hospital to home.

You will drive service improvements, supervise junior staff, and travel between New Cross Hospital and community settings. A UK driving licence and live NMC registration are essential.

Qualifications

• NMC registration is essential.
• Experience in discharge planning and service improvement desirable.
• Full UK driving licence and access to a car.
