Job description

This exciting new role offers the opportunity to work with the General Practice and Primary Care Network teams alongside pharmacists and pharmacy technicians to support pharmacy services in general practice.

You will be part of a dynamic, forward-thinking pharmacy team delivering medicines management across a PCN, supported by a named pharmacist. The team includes supervising pharmacists, a lead clinical pharmacist, and GPs to support both the team and individual members.

Your responsibilities include managing the care of patients with chronic diseases, conducting clinical medication reviews, and proactively managing polypharmacy and multi-morbidities.

You will have opportunities to attend practice, PCN, and ICB meetings, deliver training to colleagues, mentor students and junior staff, and develop innovative services in response to local healthcare gaps. Clinical supervision and mentorship will be provided.

Main duties of the job

You will support the delivery of SMRs in the PCNs, impacting patients’ medication journeys by focusing on de-prescribing, improving care quality, and reducing medicines-related admissions. Your role includes managing long-term conditions, supporting hospital discharge prescribing, advising on polypharmacy, and helping practices develop the clinical pharmacist role. You will support the prescription team and promote collaboration among pharmacists across networks.

We seek an enthusiastic, adaptable individual with excellent clinical skills and IT competence. Good organizational skills and effective communication with healthcare professionals, patients, families, and carers are essential. You should be comfortable working independently and within multiple teams.

TPC has contracts for dermatology, minor surgery, and enhanced access services, including a Pharmacy Hub providing services like Warfarin management.

Job responsibilities

The post holder will work within their clinical competencies as part of a multidisciplinary team, providing expertise in medicines management, face-to-face medication reviews, managing long-term conditions, transfer of care, safer prescribing systems, and handling prescriptions, addressing public health and social care needs.

Key tasks include medication reviews for patients with polypharmacy, especially older adults and those in residential care, leading quality improvement and clinical audits, and managing aspects of the QOF. The role is central to enhancing care quality and operational efficiency, requiring motivation and a passion for excellent service delivery.

Support will be provided to progress through the CPPE Primary Care Pharmacy Education Pathway (PCPEP) and to develop into an independent prescriber, if not already qualified.
