Overview:

This hybrid role combines in-person home visits with remote coaching—no nights, weekends, or on-call duties.

Key Responsibilities:

  • Conduct home visits to assess safety, monitor health changes, and provide clinical coaching (including medication reviews).
  • Deliver ongoing coaching via our digital app, offering resource referrals and best practice guidance.
  • Build trusting relationships with caregivers to reduce burnout and encourage positive changes.
  • Maintain accurate, timely documentation in compliance with regulations.
  • Assist with additional projects and duties as needed.

Requirements:
Education:

  • ActiveRegistered Nurse (RN) Licensein the state of employment.

Experiences:

  • 2+ yearsof recent direct care experience with elders/adults with disabilities.
  • Background incase management, care planning, and assessments(eldercare, disabilities, or complex medical needs preferred).

Other:

  • Must be able to travel within the assigned area (valid driver’s license and auto insurance required).
  • Proficient inMS Office, case management systems, and web-based applications.
  • Bilingual skills are a plus(may impact compensation).
