Qualifications

Main area: IUC Integrated Urgent Care
Grade Salary Ranges: 35,001 - 45,000
Contract: Permanent
Hours: 37.5 hours per week (Full and Part-time, Portfolio roles, Fixed, Regular and Bank shifts, including out of hours sessions, days, lates, and nights. Shift times negotiable.)

Job Overview

This autonomous role involves managing routine and urgent patient needs within professional boundaries. Responsibilities include history taking, clinical assessments (remote and face-to-face), critical thinking, diagnosis, treatment (including prescribing where able), management, and referrals. The role focuses on providing high-quality, accessible healthcare, especially to vulnerable and marginalized groups, aiming to improve health inclusion and remove barriers to holistic healthcare.

Main Duties
  • Assess, diagnose, plan, implement, and evaluate treatments for patients with diverse needs.
  • Conduct physiological and psychological assessments, and plan care accordingly.
  • Manage patient presentations, integrating drug and non-drug treatments.
  • Support patient adherence to treatments and provide medication advice.
  • Prioritize health issues, including emergency interventions.
  • Promote health and self-care strategies.
  • Provide face-to-face and remote assessments and management plans.
  • Support other healthcare professionals and team colleagues.
  • Undertake home visits as needed.
  • Ensure safety netting and risk management.
  • Make effective referrals to ensure holistic patient care.

Qualifications & Experience
  • Current NMC or HCPC registration
  • BSc in a clinical field or equivalent experience
  • Advanced assessment and decision-making qualifications or working towards them
  • Non-medical prescribing (essential)
  • Experience in mental health assessment
  • Working towards MSc in Advanced Practice or relevant experience
Knowledge & Skills
  • Experience in primary care, minor illness, A&E, 999/111, MIU, community, or OOH services
  • Knowledge of health advice and promotion
  • Autonomous diagnosis and treatment experience
  • Experience treating all age groups
  • Experience working with domestic violence cases

Certifications
