As a Clinical Navigator GP, you will support the provision of integrated urgent care across IC24's urgent care portfolio, providing on-shift senior clinical oversight to all IC24 services. This includes Clinical Assessment Services (CAS), Urgent Treatment Centres, Acute Home Visiting, Face-to-Face Appointments, and other linked services across IC24 operational sites. IC24 provides high quality integrated urgent care for over six million people.
This role involves fostering and overseeing effective communication between operational and clinical teams, clinical management, and applying key principles of safe, effective, resourceful patient care. You will coordinate the use of NHS resources across different counties, aided by a dedicated administrative team. During clinical and system stress, you will coordinate clinicians' roles on CAS, patient dispositions, and patient queues. As the senior clinician on shift, you will handle queries concerning time-critical or challenging patients from various stakeholders, guiding clinicians to the most appropriate NHS resources.
You will provide strategic guidance and oversight to operational teams, escalating bottlenecks for review. The aim is to ensure the safest possible patient care with limited NHS resources. You will monitor and review patient queues, prioritizing those in need of urgent attention and communicating delays or changes to patients.
