An exciting opportunity has arisen for an enthusiastic, motivated pharmacy technician to join our Integrated Placement of Care Hub (IPOCH) in a new pharmacy role supporting direct patient care within the Central Cheshire Integrated Care Partnership.
The IPOCH service has expanded rapidly, supporting discharge services including hospital discharge, transfer of care, home care, and community bed-based services. This team plays a key role in facilitating hospital flow and providing admission avoidance support across the community.
The post holder will possess a range of knowledge and skills to support safe medicines management within the IPOCH team. This role requires enthusiasm, self-motivation, and a commitment to developing a new pharmacy role that enhances pharmaceutical care across health and social care boundaries, fostering excellent communication between primary and secondary care teams.
The post holder will support high-quality care for patients at home requiring medication support via the IPOCH General Nursing Assistant (GNA) service. They will work across all CCICP locations to review medications in patients' homes. The candidate must be a registered pharmacy technician and act as an accuracy checker for dispensed prescriptions following Trust procedures.
