What is an Enabler?

Independence is key to well-being. Reablement involves a multidisciplinary team supporting patients to regain independence by following personalized goal plans, involving service users to help them 'do' rather than 'do to' or 'do for' people.

Job Details and Responsibilities
Purpose

Be part of the intermediate care enablement team, demonstrating our values in all interactions, supporting independence, and engaging with patients and families.

Main Responsibilities
  • Build relationships with service users, ensuring engagement with their care plans.
  • Assist with activities such as personal care, mobility, diet, medication, and record-keeping.
  • Communicate effectively with team members and escalate issues appropriately.
  • Travel efficiently between visits.
  • Lead on-site during incidents until support arrives.

  • Maintain professional relationships and high standards of care.
  • Manage training, development, and compliance with policies.
  • Use resources responsibly and participate in team and organizational development.
  • Ensure confidentiality and accurate documentation.
  • Engage in research, audits, and continuous professional development.

  • Support patient rehabilitation using appropriate techniques.
