Job overview

We have an exciting opportunity for a Clinical Team Lead - Therapist to join our Intermediate Care Team in North Cambridgeshire and Peterborough.

The Intermediate Care Service provides care and therapy in patients' homes and care home settings, facilitating hospital discharges using a home-first model and admission avoidance.

The post holder will offer therapy-focused leadership and management to ensure effective delivery of the Intermediate Care Service across the North locality, collaborating with the Clinical Team Lead - Nursing to deliver high-quality, patient-centered care.

The role involves managing Physiotherapists, Occupational Therapists, and support staff.

Main duties
  • Provide leadership and management for admission avoidance, early discharge, and transfer of care processes.
  • Work with the multidisciplinary team to streamline processes and improve patient outcomes.
  • Manage resource allocation within the team.
  • Support patient flow and discharge planning to reduce hospital stays and maximize independence.
  • Ensure effective referral and delegation systems.
  • Foster effective communication and collaboration within the team and with partners.
  • Support professional development in line with CPFT frameworks.
  • Ensure the delivery of community-based services responding to local needs.

Experience
  • Proven experience in adult and older people's care.
