Kirklees Intensive Home Based Treatment Team is looking to recruit an enthusiastic individual to join our existing team as a Band 6 Practitioner.
The service offers rapid assessment and intensive home-based treatment as an alternative to hospital admission, allowing service users and carers to be supported in the least restrictive environment. This flexible, intensive and assertive approach to engagement aims to promote recovery, co-production, inclusion and resilience. The team is able to offer a range of focused interventions, and colleagues have qualifications/experience in the areas of Occupational Therapy, Clinical Psychology, Social Work, Medicine and Nursing.
Applicants should be registered mental health nurses, Social Worker or Occupational Therapist with experience at Band 5 or above in a mental health service for adults and meet the essential requirements for the post in the person specification. Car driver is essential to this post. A period of induction and preceptorship will be available for the successful candidate.

Main duties of the jobThe IHBT provides intensive home based treatment 7 days a week, 24 hour working as an alternative to hospital admission.
The post holder will be responsible for the triage of referrals and be responsible for assessments, care-planning and co-ordination of care delivery within a team approach.
You should be forward thinking, proactive and it is essential that you are willing to work flexibly as part of a 24 hour service, which includes shifts and working weekends. The role provides the opportunity to work across the health & social care spectrum as part of an integrated team which has links to a multi-disciplinary service. The ability to form effective professional working relationships with your team colleagues and service users is an essential requirement of the job.
You will be adaptable, committed, well organised and able to work well under pressure, along with an ability to demonstrate leadership skills and deliver supervision to colleagues. You will be committed to team working, have excellent practice skills and the ability to work flexibly and creatively to meet the needs of service users.

Functions as a member of a multi-disciplinary team.
Management of enquiries and direct referrals of people in accordance with Enhanced team operational Policy and participates in the team 24 hour duty rota including on call system (if applicable to the business continuity of the service).
Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
Responsible for acting up in the absence of the team leader
