Overview
We are looking for an enthusiastic, innovative and engaging Band 5 NMC Registered Nurse who has a passion for delivering high quality patient care within community services. You will join our Integrated Out of Hours LAP Team on a part-time basis to provide remote triage of calls and referrals made to the Integrated Out of Hours Community Nursing Team. As a triage staff nurse you will maintain professional and clinical knowledge of community nursing to resolve clinical queries over the phone and enable effective remote assessment and clinical triage. Occasional delivery of care to patients in a community setting may be required to maintain up-to-date clinical experience.
You will be based at Greyfriars Therapy Centre in Stafford. The shift you would be expected to work is 5pm until 1am, with the expectation of some night shifts.
Responsibilities
- Make appropriate decisions following comprehensive remote triage to ensure safe management, and allocate visits where needed, in response to calls and referrals received by the community nursing service.
- Determine eligibility of patients referred to the service and collect additional information to support decision making. Transfer or redirect referrals to appropriate internal or external services as appropriate.
- Liaise with patients, families, carers and other service providers to support clinical decision making and identify appropriate management of referrals and messages. This may include providing advice on self-management; transfer to other services or adding contacts for home visits or clinic appointments.
- Liaise with the nurse in charge of the appropriate nursing team to allocate urgent visits and discuss potentially complex referrals.
- Review all referrals and messages to the Community Nursing Service, including understanding areas of care such as symptom management of deteriorating patients, palliative and end of life care, wound care, bladder catheter and bowel care, recognition of deteriorating patients and potentially life-threatening conditions, and complex patients with comorbidities.
Qualifications and Requirements
- Experience of working as a Community Nurse.
- Clinical knowledge relevant to the post.
- Current UK professional registration. Applicants must have current UK registration (NMC).
