Integrated Neighbourhood Team (INT) Caseworker

Lewisham and Greenwich NHS Trust (LGT) is hosting this advert on behalf of Lewisham Primary Care Networks (PCNs). Please note that LGT is not the employer for this role.

This role is available in twoPrimary Care Networks (PCNs)in Lewisham:

  • Sevenfields Primary Care Network
  • Modality Primary Care Network

Successful candidates will be employed by one of the above PCNs and aligned to a neighbourhood-based INT. Please indicate your preferred PCN(s) or neighbourhood(s) in your application. Final placement will be based on service need and candidate suitability.

Main duties of the job

We are recruiting passionate and proactive Caseworkers to join the Lewisham Integrated Neighbourhood Teams (INTs). This is an exciting opportunity to play a key role in delivering community-based, person-centred care for residents with complex needs--focusing on holistic assessments, care coordination, and addressing health inequalities.

You'll work as part of a multi-agency team supporting residents with three or more long-term conditions, linking them to health, social care, and voluntary sector services.

Aplos Health PCN is a collaborative network of four GP practices in south and west Lewisham, serving over 50,000 patients. We deliver integrated, community-focused care, addressing health inequalities through innovative services like youth wellbeing clinics and digital hubs. Our multidisciplinary team is committed to improving health outcomes and empowering local residents.

Job responsibilities

Main Duties and Responsibilities

  • Manage a caseload of residents with complex needs using a population health approach.
  • Conduct holistic assessments and develop personalised support plans.
  • Coordinate care and referrals between GPs, hospitals, social care, and community organisations.
  • Attend MDT meetings and advocate for patients' needs.
  • Support digital inclusion and contribute to continuous service improvement.
Person Specification
Experience & Knowledge
  • Previous experience in health, social care, or community coordination roles
  • Familiarity with holistic assessment approaches and patient-centred care planning
  • Understanding of population health management and tackling health inequalities
  • Competence in completing NHS Health Checks and Diabetes 8 Care Processes
  • Knowledge of the Lewisham area and its health and care landscape
  • Experience with working with those with Learning Disabilities and SMI
Other
  • Trained in phlebotomy and health care checks.
Education & Qualifications
  • Good general education with English and Maths to GCSE standard or equivalent
  • Relevant NVQ 3 or equivalent qualification or experience
Skills & Abilities
  • Strong organisational skills to manage caseloads and prioritise tasks effectively
  • Excellent interpersonal and communication skills for working with patients, families, and multidisciplinary teams
  • Analytical skills to assess population health data and identify target groups for intervention
  • Ability to conduct person-centred assessments and create individualised care plans
  • Proficiency in IT systems and digital tools for care coordination and reporting.
