Coventry and Warwickshire Partnership NHS Trust is seeking Band 5 Primary Care Mental Health Practitioners to join Integrated Neighbourhood Teams across Coventry and Warwickshire. You will provide mental health expertise, facilitate access to suitable support and help people receive timely, person-centred care closer to home.

Qualifikationen

• GCSE English and Maths at minimum; 4 or above
• Bachelor’s degree or equivalent
• Registered professional qualification (RMN/SW/OT)
• Commitment to ongoing professional development

Aufgaben

• Embed within Integrated Neighbourhood Teams to provide mental health expertise
• Undertake mental health assessments and co-create care plans
• Deliver brief interventions and short-term support
• Identify early signs of deterioration and intervene early
• Support timely access to community services and pathways
• Collaborate with GPs, social prescribers and partner agencies
• Contribute to MDT meetings and coordinated care planning
• Promote population health and equality of access for underserved groups
