A regional health and care provider is seeking an Integrated Community Services Lead to oversee Neighbourhood Teams and support service delivery improvements. The ideal candidate will have significant leadership experience, a healthcare degree, and proven abilities in managing budgets and implementing change. We have an exciting opportunity for the right person (RGN, RMN, OT or Physiotherapist) to join our team and play a major role in the ServiceDelivery Unit leadership team, leading the Redditch and Bromsgrove Neighbourhood Teams.
The role entails the operational management of these 3 Teams which includes the line management of Band 7 Team Leads, Therapy Leads, Band 4 PA and some ACPs; budget management; Quality and performance monitoring and service improvements.
The post holder will need to take part in system calls regarding patient flow and support with the escalation of delays where needed.
Leading and monitoring Innovations around working more closely with Primary Care as part of the Accelerator pilot site to further develop an integrated service approach to care. Supporting initiatives in Frailty, AHP integration and Tissue Viability.
This role includes being part of the on-call rota for the Trust.
You will work closely with the Deputy Associate Director and fellow Integrated Community Service Leads as part of our Senior Management Team.
Accountable to the Deputy Associate Director you will support strategic leadership to clinical services operationally on a day-to-day basis and will be aligned with the following services: Neighbourhood Teams, MSK / APP Services, and our Housing, Adaptations and Equipment Services.
The post holder will be involved in an extremely exciting and challenging period of service development with the further roll out of the Ageing Well Agenda, enhanced models of frailty care and developing closer working and operational alignment with partner organisations. You will require energy and a commitment to quality improvement across this diverse portfolio.
