Who We Are:
Community Services Coordination Network (CSCN) coordinates access to services and supports for adults with a
developmental disability, and for children and adolescents with complex needs in the Southwest region of Ontario.
CSCN's dedicated staff connect, collaborate, and engage with individuals, families, service providers, and
community partners to support people in navigating the complex landscape of developmental services and
children's mental health support.

support. As a strong communicator, you value working alongside a diverse group of individuals,
recognizing and respecting their unique perspectives and lived experiences.

What You’ll Do:
As an Intake Coordinator at DSO, you will serve as the first point of contact for individuals with developmental
disabilities and their families or caregivers who reside in Windsor/Essex, Sarnia/Lambton, and Chatham/Kent
communities. Your primary responsibility will be to guide them through the eligibility process for adult
developmental services funded by the Ministry of Children, Community and Social Services. A significant part of
your role involves collecting and reviewing the necessary documents to support the eligibility process for adult
developmental services. Most communication will occur over the phone or via email, and you will also be
responsible for entering data into a provincial database. The intake coordinator will be based in a CSCN office
(London, Windsor, Owen Sound, Stratford, or Sarnia) at least one day each week, while the rest of the work will
be done remotely.

What Experience You Bring:
Ideally, you have experience working with vulnerable or at-risk populations, including adults who have
developmental disabilities, along with the following:
• Post-secondary education
• Demonstrated ability to work collaboratively with individuals, families, and multidisciplinary teams
• Strong organizational/time management skills
• Excellent interpersonal and written communication skills
• Excellent computer skills and ability to work in a provincial database system
• Proven ability to work both independently and collaboratively
• Ability to interpret legislation, government policy, and guidelines
• A willingness to work flexible hours as required

Salary Range: $50,700 - $57,362.
